Blood vs. Urine Hormone Testing — Which Is Better and Why?

If you’ve been told to get your hormones checked, you’ve probably assumed that meant a blood draw. But blood is only one option for hormone testing, and it isn’t always the right one for the question you’re actually trying to answer. In our practice, we use both methods regularly, and patients often ask us directly: blood vs. urine hormone testing — which is better and why? The honest answer is that neither one is universally better. They measure genuinely different things, and understanding what each test can and can’t tell you makes it much easier to know which one (or both) makes sense for your situation.

What Blood Hormone Testing Actually Measures

A blood, or serum, hormone test measures the amount of a hormone circulating in your bloodstream at the exact moment your blood is drawn. Blood-based hormone testing is the most established method in conventional medicine, it’s what most insurance covers, and it’s backed by decades of reference ranges and clinical research. For confirming a clear-cut diagnosis — overtly low testosterone, frank hypothyroidism, a significantly elevated or depleted hormone level — blood hormone testing is the gold standard, and for good reason.

Blood is also the more accurate method for measuring both free and total testosterone, which matters a great deal in conditions like PCOS (also referred to as PMOS). If we’re trying to confirm whether someone’s androgen levels are truly elevated, blood hormone testing gives us the more reliable number.

Where blood testing runs into limits is context. A single draw is a snapshot — one moment in a hormone’s daily rhythm. For a hormone like cortisol, which naturally rises and falls throughout the day, one blood value can be technically accurate and still miss the pattern that’s actually driving someone’s symptoms. Blood also tells you how much of a hormone is present, but not what your body is doing with it once it’s there — which is exactly where the case for urine hormone testing begins.

What Urine Hormone Testing Actually Measures

Dried urine hormone testing works differently. Rather than a single circulating value, it measures hormones and their metabolites — the breakdown products your body creates as it processes and clears hormones — often across several samples collected over the course of a day. The most well-known version of this in our practice is the DUTCH Complete test, which stands for Dried Urine Test for Comprehensive Hormones. This kind of hormone testing gives us two things blood structurally can’t: a picture of a hormone’s pattern over time, and insight into which metabolic pathway your body is using to break a hormone down.

That second piece matters more than it might sound. Estrogen, for example, can be metabolized down a more protective pathway or down pathways associated with greater risk, and two people with an identical estrogen level on a blood test can be processing that estrogen very differently. The same is true for testosterone: urine testing can show us activity through the 5-alpha reductase pathway, which is relevant in conditions like PCOS/PMOS where symptoms such as acne or unwanted hair growth are driven less by the total testosterone number and more by how aggressively it’s being converted downstream.

The research on dried urine testing has grown substantially. A study conducted with the University of Southern California’s Reproductive Endocrine Clinical Laboratory, published in the peer-reviewed journal BMC Chemistry, found strong agreement between dried urine and serum results for estrogen and progesterone metabolites, with correlation coefficients above 0.95 — a high level of agreement. A separate peer-reviewed reliability study found similarly strong agreement across a broader panel of urinary hormones and metabolites when comparing dried to liquid urine samples and shortened to full-day collection methods.

It’s worth being transparent about the limits of that evidence, too. Much of the foundational validation research on dried urine testing has been conducted or funded by the laboratory that manufactures the test, rather than by fully independent third parties, and the method is not FDA-cleared. Interpreting the results also requires specialized training that isn’t standardized the way blood panel interpretation is. None of this means the test isn’t clinically useful — we use it regularly and find it valuable — but it’s a different evidence base than blood testing’s, and it’s worth knowing that going in.

Blood vs. Urine Hormone Testing, Side by Side

It can help to see the distinction laid out plainly. Blood hormone testing gives you a precise, well-validated number for what’s circulating at one point in time; it’s the standard for diagnosis and for monitoring hormone levels once treatment is underway, and it’s the more reliable method for free and total testosterone specifically. Urine hormone testing gives you a broader picture of pattern and metabolism — how a hormone rises and falls over the day, and which pathway your body favors when breaking it down — which is especially useful for anticipating how someone might respond to treatment before it starts. Cost, convenience, and insurance coverage also differ between the two: blood draws are typically covered by insurance and require an in-person visit, while dried urine testing is usually a self-pay, at-home collection. Neither set of trade-offs makes one method the automatic right choice; it depends on the clinical question in front of us.

Where Each Test Shines: How We Actually Use Them

Neither test is better across the board. In our practice, the two tend to serve different purposes at different points in someone’s care.

Establishing a Baseline Before Hormone Therapy

Before someone starts hormone replacement therapy, we’ll often run a dried urine panel as an initial baseline. This comes up especially often with women navigating perimenopause, where hormone therapy is a common consideration. The baseline is especially useful for progesterone, estrogen, and testosterone, because it tells us more than what level someone is starting from — it tells us how their body tends to metabolize those hormones. That information helps us anticipate how someone might respond to hormone therapy, and gives us a chance to get ahead of side effects rather than reacting to them after they show up. If we can see early on that someone metabolizes estrogen down a less favorable pathway, for example, we can build a plan that supports healthier clearance from the start, rather than adjusting after symptoms appear. Monitoring Hormone Therapy Over Time

Once someone has started hormone therapy, blood testing typically becomes our primary monitoring tool. It’s fast, well-validated, directly comparable to established reference ranges, and it answers the specific question we need answered during monitoring: is this person’s circulating hormone level in the range we’re aiming for. The pattern-and-metabolite information from urine testing matters most before treatment begins and periodically thereafter; the day-to-day monitoring question is one blood testing answers efficiently. PCOS and Androgen-Driven Symptoms

PCOS (also known as PMOS) is a good example of why we don’t treat this as an either/or decision. Blood testing gives us the more accurate free and total testosterone numbers, which matters for confirming the diagnosis and tracking treatment response over time. Urine testing adds the piece blood can’t: insight into 5-alpha reductase activity, which helps explain why some patients experience androgen-driven symptoms like acne or hirsutism even when their total testosterone doesn’t look dramatically elevated on paper. Neither Test Is “Better” — They Answer Different Questions

If there’s one thing we want patients to take away, it’s that this isn’t a competition between two tests where one wins. Blood testing answers “how much of this hormone is circulating right now, and is it in a normal range.” Urine testing answers “what is this person’s body doing with these hormones over time, and through which pathways.” Those are different, complementary questions, and the right test depends on which question actually matters for your care at that moment.

This is also why we’re cautious about generic advice on this topic. A blanket recommendation to always use one method over the other ignores the fact that the two tests aren’t really competing for the same job. The right approach is individualized: it depends on what you’re being evaluated for, whether you’re establishing a baseline or monitoring an existing treatment, and what specific hormonal question we’re trying to answer for you.
