New patient naturopathic visit:
Have labs available for the doctor to review – either printed or uploaded to your Charm portal (our front desk staff can help you with this). If you have labs in another portal, please be sure you are able to log in so we can view them from your device.
Our fax # – for your other providers to fax over lab info:
Complete your intake and consent forms at least 48 hours before your appointment.
Bring in any supplements you are currently taking.
New Patient Deposit Required
We require a $100.00 deposit for holding new patient appointment time slots. If you no show or cancel within 48 business hours of your initial appointment, this fee is NON-refundable. If you cancel more than 48 business hours before your appointment, the deposit is fully refundable.
When you do keep your appointment time, this fee is applied to your initial visit charges. Note – all credit card info is kept very safe and confidential.
We have this policy because we only see 1 patient at a time and we spend 75 minutes on our new patient intakes. Please help us use this time helping people by honoring our cancellation and deposit policies.
If you schedule online, a member of our staff will contact you to collect your deposit within a few days of scheduling.
New acupuncture visit:
For acupuncture and/or cupping visits please wear loose clothing. You will keep your clothes on unless you receive cupping in which case you may need to remove your shirt for that portion of treatment.
